Age With Grace offers a variety of ways to help you and your loved one plan and acheive your goals. We are a small team that tailors our services to meet your needs. There is no straight path or “one size fits all” plan for your future or to address your current situation. We offer on-going care management and one time consultations that will educate the older adult and their family, you choose what works best for your family and budget. Below are just some of our services:
Personal Assessment
Our assessment is done in the older adult’s home, no matter what setting their home may be.
Individual Life planning
We facilitate family meetings for setting and communicating goals immediate and long term future, for all interested parties. We give you guidance and assistance in achieving those goals.
Caregiver/Family support and education
We can provide long distance care oversight for families living far from their loved one and be that additional family member for you to lean on, no matter where you live. On-going care management or one time consultations are available. Helping to educate the family of an older adult, we facilitate housing transitions and/or referrals to trusted community resources if the plan is to stay in your home.

What is it like to work with me?
Once you contact me through email or by phone, I will offer you a free telephone consultation to determine if working with Age With Grace makes sense for you. Sometimes the fit is not right for either of us but I will always give you information or direction that will help you with your current situation.
Once we both agree, this is the right path for you, our work begins! Age With Grace will set up a time to do a face to face assessment in order to get to know you and better tailor our services to accomplish your goals.
